  • worsening or clinically unstable congestive heart failure;
  • acute myocardial infarction or heart attack;
  • serious ventricular arrhythmias;
  • respiratory failure;
  • severe emphysema;
  • pulmonary emboli; and
  • other conditions that cause pulmonary hypertension.
